New Patient Form
Prior to your first appointment, the LifeCare Dental team will request that you complete a Medical History Questionnaire. This will assist your Dentist understand your medical history prior to commencing any dental treatment.
You can download a New Patient Form, complete it at home and bring it along to your first appointment or you will be requested to arrive 10 minutes prior to your appointment to complete the questionnaire at reception.
